James A. Haley (Tampa) Dietetic Internship Program

Application Information

Applications are accepted by mail during the period from January 1 through February 15. Appointments will be confirmed on April 18 and the dietetic internship class will begin in August.

In order for applications to be considered, the following must be received by the Dietetic Internship Director postmarked no later than February 15.

  1. A typed personal letter of application which is to include:
    • professional goals
    • reasons the applicant chose the profession of Dietetics
    • reasons the applicant chose this internship program
    • reasons the applicant feels he/she should be selected over other applicants
  2. A completed American Dietetic Association Dietetic Internship Application Form  
    • This form is available from the Dietetic Educators of Practitioners (DEP) website at  www.depdpg.org*.  DEP is a dietetic practice group at the American Dietetic Association.  Your college program director may also have a copy of the disk available for your use.
  3. Official transcript of all courses completed at all colleges and universities attended.   Xerox copies will not be accepted.  If the transcript of the college or university granting the degree shows transfer credits of courses and grades from other schools attended, only that transcript will be required.  If only transfer credits are shown on the transcript of the degree granting college or university, this will NOT be accepted.  Courses and grades from the Fall term are required if the applicant was enrolled during that time.

  4. One Declaration of Intent to Complete Degree and ADA-Approved Minimum Academic Requirements or one Verification Statement with the original signature of the Didactic Program in Dietetics Representative or Sponsor. 

  5. Three  Recommendation forms:  ADA Recommendation forms* for the Dietetic Internship Appointments completed by:

    • Dietetic program advisor from college or university
    • College or university science or dietetics course faculty member
    • Previous employer

References must be completed on the forms included oninternet or the "Supervised Practice Program Application Disk" and must  include the original signature of the person providing the reference.  It will be very important for the individual completing the recommendation to describe the interpersonal skills, strengths, and qualities requiring further development in the applicant.  As long as the reference writer completes the checklist on the front of the form, he or she may attach a copy of their narrative if they prefer to do this instead of writing on the back of the form.

All applicants to Dietetic Internships (DI) must participate in computer matching.  Applicants should request instructions and a mark/sense card to prioritize their DI or AP4 preferences.  Applicants should request this material from any CADE-approved  or accredited Didactic Program in Dietetics or from D&D Digital Systems.  This request should be made to allow turn around time for submitting by the February 15 postmark deadline.  There is a charge for computer matching that is due with the applicant's prioritized ranking.

U.S. Citizenship is required in order to apply for a Department of Veterans Affairs dietetic internship appointment.  Pictures of applicants are not accepted.  Interviews are not given.  Application materials do not have to be sent together in one packet.  However, it is strongly recommended that the applicant have the completed application packet reviewed by a faculty advisor prior to submitting to the program as only complete application packets will be considered.

Selection of the best qualified person is made by a committee.  Grade point average (minimum at 3.0 is recommended), previous volunteer and paid work experience, your letter of application and references are major considerations. 

There is NO application fee for this internship program.
